Puppy Boutique Contract

All puppy information and description is in the checkout. You will receive an email of all information about the puppy and a copy of information at time you pick up the puppy.

I hereby agree that the above described animal is being purchased solely as a pet for myself and/ or my immediate family. If at a later date I am unable or willing to keep this pet, I agree to first contact the above described owner and give the option to reclaim said pet at no charge. I hereby agree to care for the above described pet in a humane and responsible manner and to provide it with clean and adequate shelter , food, water and veterinary care. I further agree that said pet shall reside inside my home and shall not be allowed to roam freely. I hereby agree that this pet shall wear a collar or harness with identification tags at all times.

NOTE: WAIVER OF SPECIFIC CONGENITAL WARRANTY FOR: WE DO NOT WARRANTY FOR HYPOGLYCEMIA IN PUPPIES, AS THIS IS PREVENTABLE BY PROPER CUSTOMER CARE AND HAS BEEN FULLY EXPLAINED TO THE CUSTOMER. Puppy Boutique will only cover medication for illness on the free 7 day visit, any medical cost after the 7 days is buyers responsibility. It is the consumers right, pursuant to section 828.29, Florida Statutes, to receive a certificate of veterinary inspection with each dog purchased from a pet dealer. Such certificate shall list all vaccines and deworming medication administered to the animal and shall state that the animal had been examined by a Florida licensed veterinarian who certifies that, to the best of the veterinarians knowledge, the animal was found to have been healthy at the time of the examination. In the event the consumer purchases the animal and finds it to have been unfit for purchase as provided in section 828.29(5), Florida Statutes, the consumer must notify the pet dealer within 2 days of the veterinarian’s determination that the animal was unfit. The consumer has the right to retain, return, or exchange the animal and receive reimbursement of certain related veterinary services rendered to the animal, subject to the right of the dealer to have the animal examined by another veterinarian * Any reimbursements shall not exceed 80% of the purchase price of the animal. OPTION-The purchaser for the consideration of total in checkout has agreed to waive their warranty rights as described in Florida Statutes, 828.29(6). The purchaser has 5 days( excluding Sunday), in which if the animal is certified by a Florida licensed veterinarian that it was unfit for purchase, the animal can be returned for the purchase price of the animal. The puppy must have been by a veterinarian assigned at the time of purchase price named on the free vet visit certificate, in the first 5 days to receive the free vet visit, if found at the time to have any illness, Puppy Boutique will cover the cost for medication. After 5 business days the animal will have been sold as is, no warranty either expressed or implied. If found in the first 5 days to be unfit for sale by a licensed veterinarian, the customer has the right within 2 days of the vet visit. If a customer decides to keep the pet after the 2 days, the customer is responsible for all future vet costs and treatments. * Subject to second opinion. PUPPY BOUTIQUE MUST BE NOTIFIED OF ANY PROBLEMS WITHIN 2 BUSINESS DAYS OF VET VISIT CHECK. FAILURE TO DO SO CAN VOID ALL WARRANTIES!

Purchaser has read and understood the above. They acknowledge that Puppy Boutique a Florida LLC and / or any employees do not make any promises or guarantees as to the temperament, size, breed type, coat(fur/hair) quality, or intelligence levels of the animals that are purchased. The customer further understands that Puppy Boutique, primarily sells companion pets, and makes no promises or guarantees(expressed or implied) as to the breeding or showing abilities of the animal sold. They further acknowledge that at the time of sale, the animal that they have purchased appears to be healthy, normal animal, representative of the breed to the best knowledge and is not showing any signs of illness Any and all registration applications may take up to 16 weeks or more to be available at the store if not given at time of purchase. Please Call first to ensure that they have arrived. THE PURCHASER HAS 2 BUSINESS DAYS AFTER THE PURCHASE TO RETURN THE PUPPY FOR A NON-MEDICAL REASONS. HOWEVER A $400 REPROCESSING FEE WILL BE DEDUCTED FROM PURCHASE PRICE. AFTER 2 DAYS, THERE IS NO REFUND AVAILABLE. Purchaser has read and understood the entire above contract and will receive a copy of Florida consumer rights section 828.29(5) (on bottom of vet appointment form) at time of pickup. You also give any employee at Puppy Boutiques authorization to contact and get any health related information in regards to this puppy from any veterinary office for warranty purposes.

Good for 5 days*(Excluding Sunday) from adoption.
If your puppy starts to show any signs of illness within this time frame you must call and do a walk in within the 7 business days for us to cover medication. Please also notify Puppy Boutique of any illness that has arisen during this time for health guarantee purposes as per contract. After the 7 days, Puppy Boutique assumes no responsibility for ant vet costs. In the 7 days, we do not cover hospitalization. We only cover medication.

Thank you for the purchase of your puppy and a new addition to your family. Your new puppy comes with age appropriate vaccines. However, may still require further vaccines depending on age at time of purchase and what vaccines have been completed at this time. Most vets will recommend 3-5 sets of puppy shots and a rabies vaccine between 4-6 months of age.

We are providing you with this vet visit at no costs to you for your peace of mind and post purchase check up. At this visit if any cough, cold,or intestinal parasites are found we will cover medication at this visit. We will not cover any further vaccines, rabies vaccines, or heartworm/flea prevention after the purchase date.

(5) If within 14 days following the sale by a pet dealer of an animal subject to this section a licensed veterinarian of the consumers choosing certifies that at the time of the sale the animal was unfit for purchase due to illness or disease, the presence of symptoms of a contagious or infectious disease or the presence of internal or external parasites, excluding fleas and ticks: or if within 1 year following the sale of an animal subject to this section a licensed veterinarian of the consumer’s choosing certifies such animal to be unfit for purchasing due to a congenital or hereditary disorder which adversely affects the health of the animal: or if within 1 year following the sale of an animal subject to this section. The breed, sex or health of such animals is found to have been misrepresented to the consumer. The pet dealer shall afford the consumer the right to choose one of the following options: (a)The right to return the animal and received a refund of the purchase price, including the sale tax, and reimbursement for reasonable veterinary costs directly related to the veterinarian”s examination and certification that the dog or cat is unfit for purchase pursuant to this section and directly to necessary emergency services and treatment undertaken to relieve suffering; (b)The right to return the animal and receive an exchange dog or cat of the consumer’s choice of equivalent value and reimbursement for reasonable veterinary cost directly related to the veterinarian’s examination and certification that the dog or cat is unfit for purchase pursuant to this section and directly related to necessary emergency services and treatment undertaken to relieve suffering: (c)The right to retain the animal and receive reimbursement for reasonable veterinary costs for necessary services and treatment related to the attempt to cure or cure the dog or cat. Reimbursement for veterinary costs may not exceed the purchase price of the animal. The cost of veterinary services is reasonable if comparable to the cost of similar services rendered by other licensed veterinarians in proximity to the treating veterinarian and the services rendered are appropriate for the certification by the veterinarian.
